The philosophy of hospice care revolves around the importance of advocating the needs of the terminally ill. Hospice care neither hastens death, nor prolongs life but strives to make death a natural part of life.
Heart to Heart Hospice Mission: "The overall mission is to provide compassion and caring, while striving to enhance the quality of life for those with life-limiting illnesses and their loved ones."
Through this program, care is provided to these persons by a specially trained team of Hospice professionals.
The Interdisciplinary team includes:
- Skilled Nursing (RN or LPN, on-call 24 Hours)
- Medical Social Worker
- Home Health Aides
- Medical Director
- Trained Volunteers
- Bereavement/Clergy
- Therapy Services
The team's focus is on palliative care and pain management. Care is provided for the patient in the home or in a home-like setting.
