POSITIONS:

Patient Care Coordinator
Responsible for the coordination of all aspects of patient care. Serves as a team leader for the guidance and direction of the Interdisciplinary Team.

RN Case Manager
Provides initial as well as ongoing nursing assessments and interventions, to maximize the comfort and health of patients and families in accordance with the Interdisciplinary team's plan of care. Is responsible for communicating reports on all patients to the PCC (Patient Care Coordinator). Will take call in order to cover 24-hour care.

LVN
Provides nursing care in accordance with patient interdisciplinary plan of care, under the supervision of RN/PCC.

On-Call Nurse/PRN
Responsible for providing skilled nursing, intervention, and supportive care in patient's homes for weekend, holiday, evening, and night on-call duty in accordance with the Hospice Interdisciplinary plan of care for each patient. This position may include visits as needed during regular business hours for supplemental staffing.

Certified Nursing Assistant
The CNA is under the direction of a Case Manager and/or Registered Nurse. The CNA provides personal care services within the Policies and Procedures of Heart to Heart Hospice. Participation with the Interdisciplinary Team's plan of care is required.

Dietician
Provides dietary/nutritional counseling to the patient and family in accordance with the interdisciplinary plan of care.

Chaplain
Provides psychosocial and spiritual care to hospice patients, families, and significant others. Participates in the IDG as psychosocial and spiritual advisor. Assesses patients ongoing spiritual needs.

Community Liaison
Assumes responsibility for communication, written or verbal, with the communities served by Heart to Heart Hospice. These community resources may encompass professional and non-professional sources such as churches, community centers, physicians, health care professionals, hospice organizations, funeral homes, hospitals/medical centers, organized/legal fund raising programs. This position also includes assisting the IDG team in obtaining legal's and aiding in the bereavement process.

Volunteer Coordinator
Is responsible for the day-to-day operations of the volunteer program. This job requires planning and supervising of volunteer roles. This person is also responsible for evaluating volunteers and supervising their work. The volunteer coordinator manages community involvement with other agencies or hospice community projects.

Social Worker
Provides medical social services to the Hospice patient, family and significant others in accordance with the Interdisciplinary Plan of Care. Implements and manages the Hospice Bereavement process. Assess the psychosocial needs of the patient, family members, and significant others and report findings to the IDG Team. Coordinates the patients needs for a volunteer with the volunteer coordinator.
