The Heart to Heart Hospice philosophy of care has an emphasis on positive outcomes for our patients and their loved ones. Setting goals will enable our patients to live their lives to the fullest while maintaining their dignity nearly anywhere the patient calls home. We believe that each patient should have the ability to control the direction of their own care while they are capable, as well as after they become incapable of communicating their needs and wishes. Advance directives are available to patients for future healthcare decisions in the absences of their ability to communicate their preferences that will be used by the medical professionals. When a patient is admitted to our services, a personalized care plan is developed by the Heart to Heart Hospice care team, in correlation with the patient and the patient’s primary care physician. This care plan will address the patient’s needs and wishes, as well as reflect their values and beliefs. A schedule will be created based on the amount of care that the patients and their loved ones will need from the care team. Heart to Heart Hospice will continually review the care plan, because as priorities change, so will the level of care and services needed. Heart to Heart Hospice encourages bereavement services and counseling to patients and their loved ones to help answer any questions, manage their grief, and support them both spiritually and emotionally during the end-of-life. These supportive services may continue for up to 13 months to provide relief to surviving family members and friends. We understand the difficulty associated with losing someone you love, but hope that you will be left with positive memories and peace of mind.

The Heart to Heart Hospice Foundation was created with a general mission of promoting the quality of life for individuals and organizations dealing with life-limiting illnesses, in conjunction with providing support for their caregivers, families and other members of the community. The mission of the Foundation also includes educating the community about end-of-life care, and palliative care philosophies and programs. To create the greatest impact, the Foundation collaborates with other charitable organizations and causes related to hospice care and bereavement services. The Heart to Heart Hospice Foundation is a 501(c)(3) nonprofit organization, operated exclusively for charitable and educational purposes, which is funded by generous Donations from members of the community, businesses, religious places of worship, families and friends of our patients, and other groups and foundations in support of end-of-life care. Whether the donations are made in remembrance of a loved one, or to acknowledge a care team member who has created a positive impact, the Heart to Heart Hospice Foundation appreciates the contributions made by everyone. Partners and employees support the Foundation’s mission and are encouraged to give back to the community by contributing their time, energy, and/or capital to the Foundation. The foundation uses its charitable contributions in a variety of ways to aid patients and/or their families with the financial burdens that are associated with a life-limiting illness. Our Foundation may provide grants for final wishes, funeral and burial arrangements, and additional economic support to those in need or experiencing financial distress, as well as charitable organizations and private foundations. The Heart to Heart Hospice Foundation developed a scholarship program to encourage individuals to pursue a career in end-of-life /palliative care and/or geriatric care fields. These scholarships are made possible by contributions that the Foundation anticipates collecting throughout the year. Volunteering means something different to each of the volunteers at Heart to Heart Hospice; their reasons for being involved with our hospice programs are as generous and thoughtful as they are. The volunteers at Heart to Heart Hospice are some of the most important members of our hospice care team, making a difference in the lives of others every day. Volunteers can offer their assistance by providing compassionate support and services to patients and their families, while permitting the hospice care team to focus on quality care. The following are support and activities that may be provided by our volunteers: Patient Care – help patients with simple caregiving tasks by maintaining a clean and comfortable environment, as well as assisting them with activities of daily living, running errands, light housekeeping Companionship – visit lonely patients, listen to patients and their loved ones share memories and feelings, read and play games Comfort – help patients cope with their fears at the end-of-life and console family members Support – coordinate supportive services that provide emotional and spiritual support to patients and their families Administrative Assistance – help the staff with various office duties and special events Special Services – specialty volunteers can provide massages, bring pets to visit, and play music for patients. All of the volunteers at Heart to Heart Hospice receive training prior to assisting patients and their families. This comprehensive training includes educating the volunteers about hospice philosophy, goals, and services. They will also receive training on how to care for the terminally ill, provide grief and bereavement support, and met the needs of the patients and their loved ones. Preventative safety and procedures are discussed during training to ensure the volunteers and patients well-being.
